All heart and lung conditions are serious, but many of them can be managed effectively at home with the help of our clinical team.
When your physician orders home health care for you, our skilled and caring nurses will assess your heart and lung-related symptoms and determine your unique needs. Our occupational therapist will evaluate your functional ability and teach you how to use special techniques to conserve your energy. Working with your physician, our team will monitor your progress and modify your plan of care.

Achieves’s Healthy Heart Nurses deliver disease-specific education and self-management tools for newly diagnosed and chronic cardiovascular patients. Our credentialed clinical team utilizes "best practice" assessment tools and protocols for following clients newly diagnosed with cardiac disease, pre/post-operative cardiac surgical patients, or patients with chronic exacerbations of cardiac disease. Nurses and Therapists collaborate to achieve goals of increasing activity and endurance, medication compliance and knowledge to manage disease triggers.

Chronic Heart Failure
Our goal is to prevent you from hospitalization. Achieve provides in home nursing assessments including: medication management, blood draws, weekly weights, monitoring oxygen saturation, vital signs, diet and disease process instruction.
Achieves's cardiac nurse will assist you in making lifestyle modifications to better manage your blood pressure .You will learn about your medications and how they work, how to eat a heart healthy diet, ways to become more active, and how to prevent long term hypertensive consequences such as stoke or heart failure.
Through the use of our diseases management program, the Achieve Cardiac Team will teach you your medications, dietary interactions, and how increasing activity can help the heart get stronger, preventing stroke. You will also learn how emotional stress and anxiety make the heart work harder, which can make symptoms worse. Achieves's physical and occupational therapists will work with you to develop an appropriate exercise plan which will decrease stress strengthen your heart.

By focusing on neuromotor recovery, our Achieve Rehab Team will help you regain the ability to walk, speak clearly, and think without confusion. Our goal is for you to become independent again.
Our goal is to help you live better and feel better while living with Chronic Obstructive Pulmonary Disease. We will teach you how to manage your symptoms, what triggers to avoid, and how to prevent complications. Achieves's nurse will monitoring your respiratory status, instruct on medications and diet and monitor oxygen saturation.
